Managing Deterioration in Kent and Medway

A series of free training and support that can help care settings to improve residents’ quality of care is now available across Kent and Medway.

Led by Dr Kim Gardiner, frailty lead for Kent and Medway, and delivered by Kent Surrey Sussex Patient Safety Collaborative (KSS PSC), RESTORE2 can support homes and health professional to recognise when a resident may be deteriorating or at risk of physical deterioration, and act appropriately according to the resident’s care plan to protect and manage the resident.

RESTORE2 supports the development of a consistent language that places the resident at the centre of the discussion. This in turn enables GPs and care homes to plan care more efficiently and work with a collaborative approach towards improved outcomes.

Commenting on the initiative Kim Gardner said:

“My practice supports about 260 care home beds, so I’m acutely aware of the challenges that GPs can face around improving outcomes for our care home residents.

“My practice was finding that our care homes couldn’t necessarily tell what was wrong with a resident, and hadn’t carried out observations or looked at care planning.

“Since implementing RESTORE2 we now receive a holistic assessment of the patient, which leads to improved patient outcomes, quicker.

“This is why I’m so passionate about the RESTORE2/NEWS2 physical deterioration and escalation tool for care settings that is being rolled out across Kent and Medway.”

The tool can support care homes, nursing homes, domiciliary care and learning disability settings to effectively use soft signs and the Early National Warning Score (NEWS2).
