Our Medicines Optimisation team has put together a new leaflet that explains its work, showing how it is helping patients get the maximum benefits from their medicines while reducing waste and harm.

The KSS AHSN team is working with partners on a range of projects across the region that support this work within hospitals, GP practices, care homes and community pharmacy.

And as part of the 15-strong AHSN network it also works with colleagues across England to develop, share and spread good practice in medicines optimisation.

The guide outlines the various projects taking place across Kent, Surrey and Sussex, including Transfer of Care Around Medicines. This work aims to ensure a patient gets the necessary help and advice and continues to take the right medication once they leave hospital.
